the Consent Date/Time Stamp and Consent IP address fields display on the "/FormResponseView.asp" page but without any styling on the field labels.

What is the solution?
We need to add some styling to the "User Consented Date/Time" and "User Consented IP Address" fields. We can apply the same style that exists for the custom form fields that display on the page.

Additionally, we need to break these up and display on separate lines on the page.
